Improved Health for People with Multiple Chronic Diseases

The NHS 2018-2022 will aim to enable self-care for patients with multiple chronic conditions through integrated and accessible services, to enable them to live dignified, independent and healthy lives.

Who constitutes this priority population?

Citizen and long-term residents that have been diagnosed with more than one chronic condition(s).

Why is this population a priority?

The prevalence of people with multiple chronic conditions is increasing. This is due partly to the increasing age of the population, and also the high prevalence of unhealthy lifestyle choices, such as smoking, physical inactivity and poor dietary habits. It has been established that chronic conditions constitute a majority of the causes for mortality and morbidity in Qatar, accounting for 69% of deaths in 2014.

What is our target by 2022?

25% decrease in the 30-day re-admission rate for chronic conditions.
