​ Plans for ‘Healthy Ageing’ aim at improving coordination of care for the elderly population, providing integrated home care services and continuous rehabilitation services in the community, in addition to empowering this population group with the knowledge needed to manage and improve their physical and mental health.

We will help create conditions for older people to manage their own health and maintain their independence and dignity, providing services close to home, in order to improve their well-being and active life years.

 

Who constitutes this priority population?

Citizens and long-term residents over 60 years of age.

 

Why is this population a priority?

As people age, they tend to have higher prevalence of chronic disease and co-morbidities, physical limitations and mobility needs, mental health issues and social needs. This population is at higher risk of using multiple medications, seeing multiple care providers and receiving multiple information, and being lost within the system.


What is our target by 2022?

A one-year increase in 'Healthy Life Years' for the population aged 65 years and above by 2022.
