There is a compelling case for change and a need to enhance the provision of comprehensive Continuing Care in Qatar. It is important to recognize that in isolation, even the best continuing care service models will fall short and fail to address the healthcare system challenges in Qatar.
This chapter presents an integrated strategic framework to develop an efficient national continuing care system, integrated with primary and secondary services, that delivers coordinated, patient and family centered care.
