Final assessment registration service is an evaluation of the standards that must be met by a healthcare facility to be prepared for the operational performance according to the healthcare quality and patient safety requirements.

- Security camera Approval.
- Copy of Practitioner license copies (Temporary License).
- Appointment of medical Director/Administrative Director.
- Copy of Commercial registration or Business License.
- Copy of MME License & facility registration.
- Patient Bill of rights and responsibilities stamped by QCHP.
- Undertaking of restricting the «blacklisted/banned» from practicing in any health profession.
- Copy of Radiation Certificate (in case of using Radiation Equipment).
- Copy of Patient file template.
- Copy of Civil defense approval.
- Copy of Waste management Contract.
- Copy of Price list based on the Approved MOPH Medical Services.
- Copy of the patient registration file.
- List of services provided.
