Inspection & Narcotics - Pharmacy and Drug Control Department

Service provided by Inspection & Narcotics unit of Pharmacy & Drug Control department, through which renewal of a health facility license is approved, if the required conditions are met.

Requirements:

Application for Registration of License Renewal of Healthcare Facility
List of Facility Staff
Copy of valid medical license for all facility staff
External photo of the facility
Copy of previous license for facility
Medical waste contract
Copy of the identity card of the facility owner
Copy of commercial registration & commercia license
Number or the electricity &water.
Civil defense approval
Approval of security systems department at the ministry of Interior regarding security systems & cameras.
Undertaking of restricting the “Blacklisted/Banned” from practicing any health profession.
Undertaking form for Patient bills of rights and responsibilities.
